Correct Address For 2013 W-2

If your address is different from the address printed on the payroll check that you receive on Monday, November 25, 2013, please send the correct address information to me via email before Tuesday, December 3, 2013.

This information will be used for your 2013 W-2 tax form. This information will also be used to make any necessary changes to your Indiana county tax code. In Indiana, local tax is determined by the employee’s residence on January 1st of each year. I need to submit Indiana county tax code changes to ADP in early December so that the new county tax code is effective with the first payroll of 2014.
