Health Insurance-Coordination of Benefits

Each year Unified Group Services-UGS-sends out the Annual Coordination of Benefits-Other Insurance Questionnaire forms. If you have received this form, be sure to complete it promptly and return the form to UGS. The completed form can be mailed or you can complete the form on the Unified Website using the following instructions:
Go to www.UnifiedGrp.com and log in or create your log in.
Under Quick Links-click on “Request/changes” then select “Life Status Changes” from the drop down menu.
Select the “Coordination of Benefits-other insurance questionnaire” form link from the list of available forms.
Complete the form in its entirety and click SUBMIT at the bottom of the screen.
Remember these forms must be completed and returned to UGS each year.
